Jack is listed as dying of wounds at the 69th Field Ambulance on 1 May 1916, he had been wounded earlier in the day near Souchez. The 23rd Battalion war diary entries for the period 29th April to 2nd May 1916 are transcribed below.

Bouvigny – 29 Apr 1916: Battalion relieved 1st K R R in trenches in Souchez 2 Section on night of 29th-30th April. Battalion on left, 2nd Ox & Bucks L I, Battalion on right 1st Royal Berks
